Incident Reporting Form


 This form is to report on court incidents in Beaumont Basketball Association sanctioned games, leagues, tournaments and events. Please be thourough, accurate and factual in your report. Reports should be submitted between 24 and 48 hours of the incident. 

The President and Vice-president of the BYBA serves as members' first point of contact for disputes, complaints, and concerns. Information gathered will be kept confidential and only discussed with the appropriate parties as it relates to coming to a resolution. 

Before reporting any incident please ensure you have read both the BYBA and the ABA policies below.

 

 

 

 

BYBA Complaint and Discipline Policy
  • A 24-hour waiting period rule is required subsequent to any incident.
  • After the 24-hour waiting period, the player and/or parent discusses the incident with the Head Coach and/or Manager of the Team. If the issue is resolved, then the review is complete.
  • If the issue is not resolved, then the issue is put into writing with the “Incident Reporting Form” and submitted within 48 hours of the incident. The complaint will be reviewed and a resolution of the issue will be attempted. If the issue is resolved, then the review is complete.
  • If the issue is not resolved, then any party may appeal the decision of the Discipline Committee. An appeal fee of $100, payable to Beaumont Youth Basketball Association must be submitted with the “Notice of Appeal to the Discipline Committee” form and any additional information within seven (7) days of the written decision of the President. The submission will be forwarded to the Chair of the Discipline Committee.
  • The Discipline Committee is composed of three (3) members of the BYBA Executive committee and will include either the President or Vice President. All members of the Discipline Committee will have no personal relationship or common interest with the affected parties; and are free of any actual or perceived bias or conflict surrounding the appeal. The Discipline Committee has the ability to call a hearing to review the appeal. All affected parties are required to be advised of the hearing and have the ability to attend the hearing.
  • The Discipline Committee will investigate the complaint and provide a decision in writing to the previously established affected parties within ten (10) days of the date the office received the Notice of Appeal to the Discipline Committee form.
  • After this step the review is complete, and the decision is final.
